Coach on Your Own—Your Caregivers, Our App

Health systems are operating in a much different world than they were in early 2020. There has been an unprecedented shift in the way they deliver care—and remote patient monitoring has become a critical tool to ensure continued positive outcomes for patients.

This is especially true for caregivers whose patients have diabetes. People with diabetes make several serious health decisions for themselves every day. They rely on caregiver input to validate their testing efforts and to steer them toward better health in times of need.

In the world of remote diabetes monitoring, every bit of support helps. That's where mySugr—part of the RocheDiabetes Health Connection—comes in.

Personal diabetes coaches

mySugr is a free app that connects to a patient's blood glucose meter for automatic logging. More than that, the app is an important part of the continuum of care for diabetes patients, thanks to 1-on-1 coaching.

The mySugr coaching program is accredited by the American Association of Diabetes Educators. It meets and upholds the National Standards for Diabetes Self-Management and Support, providing the highest quality education and support for people with diabetes.

When patients sign up for mySugr, they're paired with a personal diabetes coach. Our coaches are Certified Diabetes Educators® accredited by the National Certification Board for Diabetes Educators—so they really know their stuff!

Coaching can be a great network of supplemental support. They make sure that your patients are accountable for their doctor's advice in between office or remote visits. Through their remote relationship, your patient's mySugr coach can help them set and stick to goals, track progress week to week and prepare for their next visit with your caregivers.

Coach on Your Own

Sometimes there's no substitute for a caregiver's advice. You might already have systems in place for diabetes patients, but don't yet have a remote management platform up and running. That's why we offer Coach on Your Own (COYO).

COYO is a simple way to sign up one of your own caregivers as a mySugr coach. Rather than using one of mySugr's certified coaches, patients can directly access their own providers through the mySugr app.

COYO facilitates easier remote diabetes management for both providers and patients. It lets your staff see the patient's full data snapshot without the need for an office visit. mySugr easily exports testing data in easy-to-read formats that can integrate with your system.

The flexibility to offer direct remote access to your caregivers is a feature you won't find outside of the RocheDiabetes Health Connection. Ask your caregivers to become part of the winning formula—in just 16 weeks, their patients can expect to see a 1.48% reduction of estimated HbA1c.1

Whether you Coach on Your Own or lean on our certified coaches, mySugr can be a real turning point for your system's remote diabetes management. Schedule your 15-minute demo today and start seeing results.
